Does the package include the curved metal tube top

Tube shown in top left of your picture that the cable feeds through.
If not, do you sell this separately?

Can this brake be used on the front and the rear

I appreciate that the bolts used to mount the brakes to the frame are of a different lengths but can this brake be fitted to both the rear and front. if so then I will order 2
