Does this item fit a Shimano 105 Medium Cage derailleur?
Hello & thank you for your enquiry.
Shimano data shows that item 43346 is RD-R350 series specific for short cage & hence is not compatible with 105 medium cage.
Consider Series 105 5800 if your groupset is 11 speed as SJS part 36931, Shimano y5ye98010 are specific to medium cage fitment.

I cant gain compresion in the spring. Is there any tool i can use?
We cannot say, i don't see any mention of a special tool needed in the dealer manual for this:
